If you haven’t received your flu shot yet, please visit us tomorrow, Thursday, Oct. 22, at the WSU Bookstore between 9:30 a.m. and 12:30 p.m. or make an appointment at the Campus Health Center (CHC) by calling 313-577-5041. Submit your documentation by following the below steps: For students - Health professional students (e.g. nursing, medicine, pharmacy, physical therapy, etc.) should notify their department; each health professional school/college will have a documented process for uploading and verifying that students have had the flu shot.
- All other students must notify the CHC by uploading documentation or written documentation of exceptions to the CHC Patient Portal. To upload documents:
- Go to health.wayne.edu
- Click on the link at the bottom left of the page entitled Patient Portal under the Useful Links section.
- Students must create a username and password the first time they use the system.
- Once in the portal, go to the Ask a Question section
- Click on Medical Record Request
- Subject Line: FLU 2020
- Attach your document
For employees - Faculty in the health professions colleges (School of Medicine, College of Nursing, and the Eugene Applebaum College of Pharmacy and Health Sciences) should follow the usual process in their college for documentation of the flu vaccine.
- All other faculty and staff must email their documentation directly to their Human Resources consultant and to the CHC (campushealth@wayne.edu).
- If you obtain your flu vaccine from the CHC, you must only provide a copy of your medical documentation to your HR consultant.
