Greetings! Congratulations on your successful match! 

In preparation for graduation, we have two items we would like for you to complete.  

• Residency programs require us to submit paperwork certifying that you have met all the requirements for graduation. Please send your completed forms to records@med.wayne.edu 

  •  Many programs require you to download your own form from the state you have matched in. Your programs will typically provide onboarding instructions. 

  • If a form indicates that they need a Dean's signature, please send it directly to the SOM Records and Registration office. The SOM Registrar is an authorized signatory. 

• In preparation for graduation, please complete this form to let us know how you would like your full legal name on your diploma.

  • Medical School Diplomas are legal documents and must have your official legal name on it. You can adjust how your middle name is displayed.  

  • Please provide consent for WSU School of Medicine to receive information about your first year of residency. We use this information to improve the student experience. 

  • The deadline for completion is March 29, 2024.
